OSF MyChart offers 24/7, secure access to your electronic medical record. This includes hospital or outpatient visits, telephone conversations and other personal health information. With OSF MyChart, you can view your medical history, test results, clinical notes and more.
You can also view your health information from other organizations by linking to an existing MyChart account.
Want to ensure that a trusted friend or family member can view your medical record? OSF MyChart can help with that, too! You may grant or revoke proxy access at any time, plus you can even share your information with employers or providers in other health systems.
Note: Individuals with proxy access will be able to view the same personal health information that you are able to access within your own individual account. Because your medical record contains information that may be sensitive in nature, we recommend that you periodically review who has access to your account.
